Leg length discrepancy means one leg is measurably shorter than the other. It can be structural (the bone is shorter) or functional (the leg is the same length but pelvis tilt makes it act shorter). Either way, the body compensates to keep the eyes level — and that compensation runs all the way up the spine.

For people with a clinically significant difference — generally above 2 cm per AAOS guidance — daily compensation means uneven shoe wear, one-sided knee or hip pain, recurring lower back ache, and tired legs at the end of the day. A leg-length gap can be structural or functional. The way to spot it — and what to do about it — depends on age.

What to look for in children

  • Visible difference in shoulder or hip height when standing barefoot.
  • One trouser leg consistently dragging more than the other.
  • Limping during long walks or sport, especially when tired.
  • Tilted head or curved standing posture (compensating for the difference).
  • Persistent one-sided knee, hip, or back complaints with no injury.
  • Foot of the shorter leg often turns out slightly to compensate.

Why it matters during growth

Most kid leg-length differences are under 2 cm and resolve as growth equalises. Anything above 2 cm, or any difference causing pain or visible posture issues, is worth assessing.
